Family Surrogacy

We interact with an individual to establish solid rapport, assist with current difficulties, build a trusting relationship, and insure that carefully planned care is delivered over time.

Case Management

Once a Care Plan is developed, PLAN commits to coordinating the plan for the future. Essentially, PLAN becomes a surrogate family member for a person with a disability. PLAN is a surrogate who has an established relationship with that person, a comprehensive vision of his/her ideas about the future, and detailed knowledge of his/her current issues. PLAN sees that the plan is implemented as prepared, changed as needed, and is updated regularly. As family surrogates, we interact with an individual to establish solid rapport, to assist with current difficulties, and to build a trusting relationship over time and ensure that carefully planned care is delivered over time.
